1. What are some of the barriers and limitations to the transition of one level of care to another?

First, it is important to have a case manager for patients and organizational purposes to make these transitions as smooth and free from limitations as possible. For example, a compassionate, professional care manager (or a number of them at each level of care) is usually responsible for the assessment and referral process and all treatment authorizations. In other words, the process is integrative, and thus, cooperation and coordination at all levels of care is imperative. This is one area where barriers and limitations in the system have been reported.

By providing easy access to care, matching patients with the most appropriate providers, and coordinating and facilitating the transition from one level of care to the next, each level of the health care system, through a care manager(s), often helps deliver more positive outcomes.
